JOIN THE PATIENT PARTICIPATION GROUP
If you would like to join our Patient Participation Group, please ask at Reception for a Registration Form - or you can download, print and complete the form below.
All registered patients are welcome to join the PPG although, in the interests of effective decision-making, membership of the core is limited to twelve. Specialist sub-groups may be formed to deal with specific topics utilising individual skills and interests.
The PPG meets regularly - approximately every 4 to 6 weeks.
The Group will NOT represent any patient with a complaint concerning medical treatment received from the Practice - or other NHS service provider(s) at the Health Centre. There is a standard procedure for Practice complaints which is available from the Practice Manager.
The Group is 'non-party in politics' and 'non-sectarian in religion'. It will, at all times, respect diversity and will be committed to the principles contained within the Equality Act.

Membership
The Group will comprise of up to 12 members. Any patient may nominate themselves by completion of the application form. Removal of a patient from the Health Centre List, for whatever reason, will disqualify continuing membership of the Group. Membership of the Group is open to all patients over the age of 16 who are patients of the Health Centre. (Verification will be required). |
